Surgical and traumatic scars may benefit from early laser intervention. The pulsed dye laser (where there is a dominance of erythema) and/or fractional ablative or non-ablative treatments have been shown to be efficacious. A particular issue is with scars that will be predictably poor (site, genetics, lacerations etc) or scars which develop poorly in unpredictable ways.
Often traditional interventions with silicone applications, pressure regimens (digital devices or masks) and early intralesional medical treatments are unsatisfactory.
‘Q’ switched and pico-second lasers deliver high fluence via ultra-short energy pulses which shatter chromophores and may nonspecifically stimulate deposition of collagen and increase thedensity and length of elastin fibres to help normalise scar tissue.
