The human papilloma virus causes very persistent warts and verrucae in many individuals. Targeting the vascular component (pulsed dye laser, Nd: YAG 1064nm laser) can be effective.
Topical keratolytics may be required to reduce hyperkeratosis and on occasion, particularly in the case of immune suppressed individuals, systemic retinoids may be indicated to enable vascular lasers to be effective.
For extensive mosaic, hyperkeratotic lesions, the CO2 laser (collimated handpiece) is excellent for controlled planing to papillary or deep dermal layers and ablation of warts with good haemostasis. The smoke plume is an issue and HEPA filtered smoke extractive equipment is essential. Immunosuppressed patients are a particular challenge.
